Discharge to assess (D2A)

Powered by:

Bringing longevity, quality of life, and social connectivity to every older adult with an AI-augmented experience.

MiiCare is heading a consortium that has successfully sought and obtained an Innovate UK Smart grant to address the challenges connected with the discharge to assess model. As part of this grant, we formed partnerships with De Montfort University, Connected Places Catapult, Leicester, Leicestershire, and Rutland ICB, and Willows Health.

Delayed discharges cost the NHS £820 million annually.

With longer hospital stays associated with infection risks, permanent cognitive decline and more…

MiiCare and the consortium aim to utilise our AI-based conversational technology to support the discharge-to-assess process. By introducing unconventional data collection and analysis methods, we will be able to offer valuable health insights alongside established clinical data sources like blood pressure and heart rate. Helping to assess the overall well-being of patients in their home environment post-discharge.

Our Solution

The proposed solution encompasses the following key features:

Our bespoke AI system will

Automate and schedule patient care assessments with Monica.

Use conversations with Monica to evaluate mental health status and detect early indicators of mental health conditions.

Reduce number of patients needing to be discharged to a 24-hour residential care setting.

The detection of possible symptoms is made possible by recognising changes in voice patterns.

Overall patient’s well-being through data collected from Monica, and the other IOT devices.

Provide 24/7 interactive virtual support to users that can help reassure them and their families.
